2. Categorize test info under:

                                                         General Laboratory Test

                                                (FBS, 75g OGTT, and Lipid Profile)
                                                         ▪ 8-10 hours fasting
                                                         ▪ No water intake
                                                         ▪ *Overfasting if more than 10 hours (you may opt to proceed with testing)

 3. Add Semen Analysis

Patient Instructions:

              ▪ Abstain from sexual activity (including masturbation) for at least 3 to 5 days prior to your test date. This ensures that the test is as accurate as possible.

              ▪ Please confirm your testing appointment one day prior to your test date.
